Case management services for abused/neglected and self-neglected elderly, early intervention, community and home care, general assessments, support groups, caretakers, and victims of abuse, pharmaceutical assistance, and Meals on Wheels. Available to seniors ages 55 years and over and adults with disabilities and their families or caregivers residing in the towns of Charlestown, Exeter, Hopkinton, Narragansett, New Shoreham, North Kingstown, Richmond, South Kingstown, Westerly and West Greenwich. This program helps seniors and disabled adults who are eligible for RI Medical Assistance and in need of Personal Care through the development, implementation and monitoring of their Individual Service and Spending Plan (ISSP). ISSPs promote independence by providing assessments of accessibility, equipment, and health education needs and assistance arranging necessary services.
Referrals are accepted on a statewide basis.

Coordinates health care fraud, waste and abuse training sessions and presentations in local senior centers, assisted living units, elderly housing units and the community in general; and to assist Medicaid and Medicare beneficiaries to help them navigate the state system.
